Job Description

Join us as a Case Executive and be part of our CREST-PDS team.

You will need to:

• Provide case management support to clients who are newly diagnosed with dementia and their caregivers
• Provide care coordination and service linkage to clients with dementia and their caregivers, with the appropriate resources and support including caregiver support
• Participate in multi-disciplinary case discussions with hospital professionals relating to the well-being of clients who are diagnosed with dementia
• Conduct regular home visits to monitor the well-being of the caregiver and client with dementia
• Support the evaluation and documentation of the programme
• Support other ad-hoc assignments as assigned by CREST-PDS Lead/Department Head

• Have good knowledge of the geriatrics and community services
• Be proficient in Google Suite applications
• Have strong communication and stakeholder management skills
• Be passionate about community services and be service-oriented
• Have good assessment and decision-making skills
• Be meticulous and enjoy working independently

Qualification

• Degree/Diploma in Social Work/Psychology/Counselling
