Responsibilities

• Conduct thorough assessments of patients’ medical, psychological, and social needs utilizing EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems
• Develop and implement personalized care plans aligned with clinical guidelines
• Coordinate with facility's Interdisciplinary Team (IDT)—to ensure effective discharge planning and utilization review
• Monitor patient progress through ongoing communication with healthcare providers, social workers, behavioral health specialists, and family members
• Manage intake processes for new patients requiring complex case management services in post acute care settings
• Ensure compliance with HIPAA regulations by maintaining confidentiality of patient information at all times
• Assist in utilization management by reviewing medical necessity criteria and supporting documentation for appropriate resource allocation

Experience

• Familiarity in discharge planning, utilization review, and managed care processes
• Knowledge of medical terminology and utilization management standards
• Must possess the ability to make independent decisions, follow instructions
• Skilled in navigating EMR/EHR systems while maintaining HIPAA compliance
• Ability to generate written communication that is clear, concise, and well-organized
• Excellent oral communication skills to collaborate effectively across multidisciplinary teams in fast-paced hospital settings
• Experience working in Skilled Nursing (SNF) settings preferred

Pay: $19.00 - $22.00 per hour
