Responsibilities
Provide case management services to clients in need of support and guidance
Conduct overviews to determine caregiver logs according to the time and care plan approved,
Coordinate and monitor services to ensure clients' needs are met effectively
Offer crisis intervention and support during challenging situations
Experience
Previous experience in Home Care or related field is required
Proficiency crisis management strategies
Strong understanding of Home Care regulations and guidelines
Experience working with the elderly or providing personal care services is a plus
