Qualifications:·
- Prior experience as a case manager in Assisted Living, Nursing Home or other residential settings preferred.
- Able to work compassionately with Seniors
- Good writing skills, able to document case notes
- Able to develop rapport with residents, families and those involved in the care of the resident
- Good communication and problem-solving skills
- Bachelors degree-preferred
