Case Manager

Summary

Under the direction of the Division Director, the Case Manager will implement linkage and individual rehabilitation services for participants of SSG Weber Community Center. The Case Manager will provide office or field-based individual, group and family needs assessments, linkages, individual rehabilitation and follow-up services. The Case Manager will also conduct outreach services to promote agency services.

Essential Functions

• Conduct needs assessment and client service plan that meets individual needs, securing needed services for participants, and meeting with participants regularly to discuss their progress
• Participate in weekly case conferencing with treatment team
• Provide comprehensive, professional case management services to participants through encouraging, promoting and developing positive life skills and meeting emergency and long-term participant needs.
• Conduct outreach services to identify and engage potential participants.
• Conduct outreach to housing networks and employment and life skills development networks where linkages can be created.
• Maintain up-to-date and accurate documentation in client files.
• Collaborate with our partners as well as any other support in the community.
• Complete all documentation within the standards of the Department of Mental Health in a timely manner.
• Maintain a minimum of 55% billing productivity monthly.
• Billing expectation of 22 hours weekly of DMH Billing.
• Assist with the completion of necessary client documentation (i.e. consent forms, etc.)
• Maintain files/records on client services in compliance with HIPAA and other funding requirements for auditing purposes.
• Work on other appropriate tasks assigned by Supervisor and Managers
• Other duties as needed
• Regular attendance required.

Secondary Functions

Non-traditional hours, overtime, on-call, holiday, or weekend work may be required.

Minimum Qualifications - Knowledge, Skills and Abilities Required

• Strong organization, communication and teamwork skills, and ability to handle crisis situations.
• Computer Literacy – ability to use MS Office, Outlook, Adobe programs, Electronic Medical Software, and any other relevant computer programs that are necessary to fulfill required job duties.
• Valid California driver license, Proof of Car Insurance, and Reliable Transportation.
• Verification of Employment Eligibility and Background check.
• Bachelor’s degree in social work, psychology, human services or related field experience.
• Ability to organize and manage multiple priorities.
• Maintain and uphold Agency mission statement, values, policies, procedures, and principles.

Supervisory Responsibilities

· This position does not have any supervisory responsibilities.

Environmental Conditions (Working Conditions)

· The environment for this position is field based as well as an office environment. It involves working within at-risk areas including home and school-based services. Exposed to potentially highly aggressive clients.

Physical Requirements

· Typically spends time sitting, standing, walking, driving, carrying (max. 20lbs), listening, speaking.

Mental Requirements

· The incumbent in this position must be able to handle any/all of the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules; be able to process information, think, and conceptualize.

Pay: $25.00 - $30.00 per hour
