Case Manager

LifeStyles of Maryland Foundation is seeking to hire a full-time Case Manager, to assist households in emergency crisis situations and transitioning them to more sustainable situations. The Case Manager will work with both those that are experiencing homelessness and those that are in need of homeless prevention efforts

Responsibilities:

• Completes client Intake and pre-screenings for emergency financial assistance, financial literacy, health services and related gap-filling requirements
• Monitors client assigned cases to assure the compliance with legal/judicial proceedings
• Serves as an invaluable resource for the attorney for the client/family and social worker in reaching goals for permanency or stability
• Investigates, researches and interviews clients or related resources; completes an intensive independent review of cases
• Attends hearings and related court case meetings
• Provide in-person assistance with clients and assigned to various locations throughout the agency's Southern Maryland or Prince George's County offices
• Attends inter-agency meetings regarding client/family; collaborates and coordinate with multiple agencies and organizations
• Provides timely case tracking and follow-up on client assessments and ensure client files are maintained with accuracy and confidentiality.
• Assesses the needs of the individual or family including their basic needs such as food, clothing and shelter.
• Ensures that referrals are made to appropriate internal and external services as required to meet the client and/or family’s needs
• Provides verbal and written reports containing independent, factual information on assigned cases
• Performs follow-up on referrals to ensure individual and/or family responsiveness to all referrals made by LifeStyles and related organizations within program.
• Complete data entry for Charity Tracker and any other approved data management system required

Qualifications

• Minimum of 2 years experience in related field: Health and Human Services, Social Work, Counseling or experience working with families, service planning, crisis intervention or emergency help desk.
• Education: BS/BA Degree or higher; supplemental course work in an accredited or related field
• Valid Maryland Driver's License and reliable transportation to and from work
• Excellent communication, both oral and written; interpersonal and team-player; strong computer skills including document management
• Ability support outreach functions on evenings or weekends
• Ability to pass background check and drug screen
• Spanish-speaking capabilities is preferred'
