What You Will Do

• Identify, assess, and plan care strategies for patients, ensuring their health care needs are met.
• Facilitate care coordination and advocate for patients and their families.
• Provide discharge planning services to support safe transitions to the next level of care.
• Manage and address the needs of patients with critical diagnoses or events.
• Collaborate with the healthcare team to promote cost-effective and high-quality outcomes.
• Utilize community resources to support diverse patient populations.

What You Will Need

Required Qualifications

• Bachelor's degree in nursing or Master's degree in Social Work.
• Minimum 3 years of clinical experience.
• Excellent verbal and written communication skills.
• Strong customer service and interpersonal skills.
• Proficiency in problem-solving, analytics, and decision-making.
• Computer literacy and knowledge of healthcare systems.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid Michigan license.

Preferred Qualifications

• Experience in discharge planning, home health care, or managed care.
• Certification in Case Management (CCM) or Accredited Case Manager (ACM).
