Case Manager

About the Role:

Manna Of Life Ministries Inc is looking for a compassionate and dedicated Case Manager to join our mission-driven team in the Bronx, NY. This is a meaningful opportunity to make a real difference in the lives of individuals and families by connecting them with the resources and support they need to thrive.

Responsibilities:

• Conduct client intake assessments to identify needs, goals, and eligibility for services
• Develop and implement individualized care plans in collaboration with clients and support teams
• Connect clients with community resources, social services, and referral networks
• Monitor client progress and adjust service plans as needed to ensure positive outcomes
• Maintain accurate and up-to-date case documentation and client records
• Advocate on behalf of clients to ensure access to housing, healthcare, and social support services
• Collaborate with community partners, service providers, and ministry staff to coordinate care

Requirements:

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field
• Prior experience in case management, social services, or community outreach preferred
• Strong knowledge of Bronx/NYC community resources and social service systems
• Excellent interpersonal, communication, and active listening skills
• Ability to work with diverse, at-risk, or underserved populations with empathy and respect
• Strong organizational skills with attention to detail in recordkeeping and documentation
• Bilingual (English/Spanish) a plus
• CARES Experience Preferred
