We are seeking a dedicated and compassionate Case Manager to join our healthcare team. In this vital role, you will support patients across multiple specialties by coordinating their care, facilitating access to necessary services, and ensuring seamless continuity throughout their healthcare journey. Working under the guidance of the Director of Clinical Quality, you will play a key part in bridging gaps in care, reducing barriers, and promoting patient-centered outcomes to enhance overall quality of care.

Key Responsibilities:

• Coordinate patient care across multiple specialties to ensure comprehensive and continuous treatment.
• Facilitate access to healthcare services, community resources, and support programs tailored to individual patient needs.
• Act as a liaison between patients, healthcare providers, and community resources to promote effective communication and collaboration.
• Monitor patient progress and adjust care plans as needed to meet evolving health requirements.
• Document and maintain accurate records of patient interactions, care plans, and outcomes.
• Identify and address barriers to care, including social, economic, or logistical challenges.
• Educate patients and their families about treatment options, medication management, and available resources.
• Collaborate with healthcare teams to develop and implement patient-centered care strategies.
• Ensure compliance with healthcare regulations, policies, and quality standards.

Skills and Qualifications:

• Bachelor's degree in Nursing, Social Work, Healthcare Administration, or related field; relevant experience may be considered in lieu of degree.
• Proven experience in case management, care coordination, or a related healthcare role.
• Strong understanding of healthcare systems, community resources, and patient advocacy.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work collaboratively with multidisciplinary teams.
• Compassionate approach with a focus on patient-centered care.
• Knowledge of healthcare regulations, privacy laws, and documentation standards.
• Ability to manage multiple priorities and adapt to changing healthcare environments.
