Case Manager

Duties:

• Provide direct and indirect patient care in a variety of healthcare settings
• Perform assessments, develop care plans, and implement nursing interventions
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report any changes or abnormalities
• Provide education and support to patients and their families on various healthcare topics
• Collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes
• Document patient care activities accurately and timely

Qualifications

• Minimum of 2 years of experience as a licensed Registered Nurse

• Experience in Hospice or Home Health Care preferred

• Proficient in planning and coordination of care for patients transitioning to home or other healthcare facilities

• Ability to navigate sensitive conversations with empathy and clarity

• Knowledge of Durable Medical Equipment (DME) and its usage in patient care

Requirements:

• Valid Registered Nurse license in LA
• Current CPR certification
• Strong communication and interpersonal skills
• Ability to prioritize and multitask in a fast-paced environment
• Attention to detail and ability to maintain accurate documentation
