Case Manager

Job Summary:

Case Manger is responsible for maintaining a safe and structured environment for women and children residing at Redwood Family Center. The Case Manager demonstrates a commitment to providing the highest quality care and supervision to the clients being served. To ensure that quality exists, employees must display good moral character, emotional stability, and possess sufficient health, ability, experience and education to perform the duties assigned.

Typical Tasks:

1. Maintaining and keeping up-to-date accurate records on individual clients, including passport, encumbrance, in-take, case notes, ASI, clothing inventory, daily shift log, medical and TANIF application

2. Meeting with clients once a week which includes investigating medication taken properly, drug testing, financials are in order, mental health is being addressed

3. Walk through the facility at the start and end of each shift and logging any results

4. Ensure residents receive medications as prescribed by the physician and adhere to VRR policy and procedures

5. Drug test on regular basis with proper documentation

6. Observe CPS visits and facilitate activities

7. Assist in transportation of residents as directed by Program Manager

8. Maintain vehicle records/mileage logs

9. Ensure household routines are followed by coaching/modeling and adjusted when necessary – chores completed and signed off

10. Oversee meal planning, meal preparation and shopping for the facility

11. Maintain documentation of expenditures and adheres to financial policies of VRR

12. Participate in staff training and meet with Program Manager as schedules

13. Assist the Program Manager in conducting intake assessments

14. Facilitate case load graduation

15. Continue to case manage graduate house clients by-weekly
