Position Overview

MINT Physical Therapy is seeking a Case Manager to support patients throughout the course of their physical therapy care. This position focuses on coordinating patient cases from referral through discharge, managing scheduling, communicating with referral sources and healthcare providers, and working closely with physical therapists and other case managers to support positive patient outcomes.

MINT Physical Therapy provides clinic, mobile, workplace, and home-based care throughout Northern Utah. The organization specializes in workplace injury prevention, workers’ compensation, motor vehicle accident rehabilitation, and a variety of specialty services.

Key Responsibilities

• Manage patient cases from initial referral through discharge.
• Coordinate patient care and schedule appointments.
• Communicate with referral sources, healthcare providers, patients, and members of the care team.
• Coordinate with physical therapists and other case managers to support optimal patient outcomes.
• Help maintain organized and timely follow-through on patient cases and referrals.
• Work effectively across clinic, mobile, workplace, and home-based care settings as needed.

Compensation & Benefits

• Full-time position.
• $16.00–$22.00 per hour- Low end of pay range for individuals lacking experience that need to start off in a front office/reception role.

Work Environment

The role will begin initially in Riverton, and then after a few months will be stationed in Utah county, most likely Provo.

Pay: $16.00 - $22.00 per hour
