CDLF serves communities across Los Angeles, Riverside, San Bernardino, and Orange Counties, focusing on individuals who have been marginalized or de-prioritized by the traditional mental health system. Its mission is to deliver trauma-informed mental health services and resources to children, adolescents, and adults who need them most.
Role Description The Case Manager is a full-time, on-site role based in Santa Ana. This role is responsible for coordinating and monitoring client services, including intake assessments, service planning, and referrals to appropriate mental health and social service resources. The Case Manager will maintain regular contact with clients, track progress toward treatment and support goals, and collaborate closely with therapists, counselors, and community partners. Daily tasks include documentation and recordkeeping, advocacy on behalf of clients, crisis support as needed, and participation in team meetings and case conferences.
