Case Manager – reMIND Behavioral Health

As a Case Manager, you’ll put that mission into practice by supporting adults receiving PHP, IOP, and outpatient treatment as they work toward stability, independence, and lasting recovery.

Your scheduled workday is paid, not just direct client contact! Employees are paid hourly for client support, care coordination, meetings, and administrative time. Documentation time is built into the schedule, and we provide AI-assisted tools to help with documentation. You review and personalize your documentation before finalizing it.

Focus on clients, not calendar management. Our scheduling team fully manages provider calendars, including scheduling and rescheduling client appointments. Typical employee hours are 8:00 a.m. to 4:30 p.m. as the position grows. This role will begin part-time, with the opportunity to transition to full-time as our client census grows.

What You’ll Do

• Help clients identify barriers to recovery and develop practical plans to address their needs.
• Connect clients with community resources, housing support, transportation, employment services, benefits, and other essential services.
• Coordinate care with treatment providers, community partners, and other members of the client’s support team.
• Support clients in navigating treatment recommendations, appointments, and recovery-related goals.
• Monitor client progress, identify emerging needs, and communicate concerns to the appropriate clinical team members.
• Maintain timely, accurate documentation of services, client interactions, referrals, and progress.
• Participate in team meetings and collaborate with clinical staff to support continuity of care.
• Maintain professional boundaries, confidentiality, and a trauma-informed, person-centered approach.

We’re looking for a Case Manager who communicates clearly, builds positive relationships, and is passionate about helping individuals overcome barriers and build a foundation for lasting recovery.

• Experience in behavioral health, substance use treatment, case management, or a related human services field is required
• Bachelors Degree is required.
• Strong organizational, communication, and documentation skills.
• Ability to work collaboratively with clients, clinical staff, and community resources.
• Compassionate, dependable, and able to maintain professional boundaries.
• Familiarity with substance use recovery, co-occurring mental health needs, and community-based resources is a plus.
• Willingness to learn applicable documentation, treatment, and care coordination requirements.

Compensation & Employment

• Pay: $22–$25 per hour, depending on experience and qualifications.
• Begins part-time, with the opportunity to grow into a full-time position as the client census increases.
• Work hours will be coordinated based on client needs and program growth.
• Benefits eligibility is subject to applicable company policies and employment status.

Benefits are available to employees working 30 hours or more per week, subject to applicable plan requirements.
