Qualifications

To be considered for this position, applicants must meet at least ONE of the following qualifications:

• Possess a four-year degree in a person-centered or related human services field; OR
• Have at least five years of experience working in some capacity within the I/DD Waiver field. (any experience will be considered from house keeping, direct care, to admin your encouraged to apply)

We recognize that valuable knowledge and skills can be gained through both education and hands-on experience.

Job Responsibilities

As an I/DD Waiver Case Manager, you will:

• Advocate for individuals receiving I/DD Waiver services.
• Coordinate and monitor essential supports and services.
• Facilitate person-centered Individualized Program Plan (IPP) meetings.
• Collaborate with members, families, guardians, providers, and other professionals.
• Complete required documentation, home visits, and service monitoring.
• Assist individuals in achieving their personal goals and greater independence.
• Conduct required quarterly planning meetings and home visits in members' homes.
• Maintain accurate and timely documentation in accordance with program requirements.
