Job Overview

We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. Duties

• Conduct thorough patient assessments, including intake interviews and social services evaluations, to develop personalized care plans.
• Collaborate with multidisciplinary teams departmen
• Manage utilization review and utilization management processes to ensure appropriate resource use while complying with healthcare regulations like HIPAA.
• Facilitate discharge planning by coordinating with partner programs and community resources to ensure smooth transition to other care settings.
• Maintain accurate documentation within EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems, including medical terminology, CPT coding, ICD-9/10 coding, and ICD coding for billing purposes.
• Communicate effectively with patients and their families as necessary.
• Support hospital initiatives such as Level I and Level II trauma center protocols, managed care programs, and patient service improvements.

Qualifications

• Knowledge of Substance Use and Behavioral Health setting
• Strong knowledge of medical terminology, CPT coding, ICD-9/10 coding systems, and utilization review processes
• Familiarity with EMR/EHR systems and Microsoft Office applications for documentation and reporting purposes.
• Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds.
• Knowledge of HIPAA regulations and health insurance processes to ensure compliance and confidentiality.
• Bilingual abilities are a plus to enhance communication with a broader patient demographic.

Pay: $23.00 - $25.00 per hour

Expected hours: 40.0 per week
