We are seeking a dedicated and compassionate Case Manager to coordinate services and support for residents of Insight Disability Services (IDS). This role involves managing an assigned caseload across various service lines, including Serious and Persistent Mental Illness (SPMI), Traumatic Brain Injury (TBI), and Intellectual and Developmental Disabilities (IDD). The Case Manager reports to the CM RN Supervisor and collaborates closely with residential leadership, the Medical Services Division, and other stakeholders to ensure comprehensive, person-centered care. The position plays a vital role in facilitating admissions, coordinating medical appointments, maintaining accurate documentation, conducting quarterly reviews, and planning for discharge.

Duties

  • Coordinate services and supports for residents within the assigned caseload, ensuring individualized care plans are effectively implemented.
  • Manage admission and intake processes for new residents, ensuring smooth transitions into the program.
  • Facilitate coordination of medical appointments and follow-up to support residents’ health needs.
  • Maintain accurate and timely documentation of all case management activities, progress notes, and service plans.
  • Conduct quarterly reviews to assess resident progress, update care plans, and adjust services as needed.

  • Collaborate across all IDS service lines—SPMI, TBI, and IDD—working closely with residential leadership and medical professionals to deliver integrated support.
  • Serve as a liaison between residents, families, healthcare providers, and community resources to promote holistic well-being.

Experience

  • Prior experience in case management or a related role within healthcare or social services is preferred.
  • Strong understanding of mental health, developmental disabilities, or brain injury populations is advantageous.
  • Excellent organizational skills with attention to detail for documentation accuracy.
  • Ability to coordinate multiple services effectively while maintaining compassionate communication with residents.
  • Knowledge of community resources, healthcare systems, and relevant regulations enhances success in this role.

Minimum two (2) years of experience in human services, case management, healthcare coordination, or social services.

Minimum twenty-five (25) years of age. This requirement is established by the minimum driver age condition of the IDS commercial automobile insurance policy and applies to all positions authorized to operate company vehicles.
