Case Manager

SkyFall Behavioral Health is seeking a compassionate, motivated, and organized Case Manager to assist clients with accessing community resources, coordinating services, and working toward individualized goals. The ideal candidate will have strong communication skills, a commitment to client advocacy, and the ability to work effectively with individuals experiencing behavioral health and social challenges.

Responsibilities

• Coordinate services and community resources to meet clients’ identified needs.
• Assist clients with accessing housing, food assistance, transportation, employment, education, healthcare, and other supportive services.
• Collaborate with clients to establish goals and monitor progress.
• Provide referrals and follow up with community agencies and service providers.
• Coordinate with families, schools, healthcare providers, and other professionals when appropriate and authorized.
• Maintain accurate and timely client records, progress notes, and service documentation.
• Protect client confidentiality and follow agency policies and applicable regulations.
• Advocate for clients and encourage independence, stability, and self-sufficiency.
• Participate in team meetings, supervision, and required training.
• Complete other duties as assigned. Qualifications

● High school diploma or equivalent required; an associate or bachelor’s degree in human services, psychology, social work, or a related field preferred.

● Experience in behavioral health, case management, social services, or a related field preferred.

● Strong communication, organization, and problem-solving skills.

● Ability to maintain professional boundaries and handle confidential information.

● Basic computer skills and the ability to complete electronic documentation.

● Ability to work independently and as part of a team.

● Reliable transportation if community-based travel is required.

Additional education, experience, or credentials may be required depending on the specific services assigned to the position.
