Case Manager

Empowering Healthcare Services is seeking an experienced Licensed Practical Nurse (LPN) Case Manager to join our growing behavioral health team!

This position is ideal for an LPN who enjoys working independently, building relationships with patients, and coordinating mental health services for older adults.

Our Case Managers play an important role in our collaborative approach to behavioral health care, working closely with psychiatric providers, facility staff, and other healthcare professionals to support patients residing in assisted living communities and skilled nursing facilities throughout the Atlanta area.

This is a hybrid position, offering the flexibility of remote work combined with visits to assigned healthcare facilities.

Key Responsibilities

• Manage an assigned caseload of patients receiving behavioral health services.
• Conduct mental health screenings and monitor patient symptoms within the LPN scope of practice.
• Coordinate patient care with psychiatric providers, facility staff, and other healthcare professionals.
• Monitor patient progress, treatment response, medication adherence, and reported side effects.
• Provide patient and family education regarding mental health conditions and treatment recommendations.
• Assist with coordinating treatment plans, referrals, and follow-up services.
• Participate in regularly scheduled collaborative care meetings and patient case reviews.
• Communicate changes in patient conditions and concerns to the appropriate clinical providers.
• Maintain accurate and timely documentation in electronic medical records.
• Build positive working relationships with patients, families, and facility staff.

Required Qualifications

• Active, unrestricted LPN license in the State of Georgia (required).
• Minimum of 3 years of nursing experience.
• Minimum of 3 years of case management or care coordination experience.
• Strong communication, organizational, and documentation skills.
• Ability to work independently and manage an assigned patient caseload.
• Comfortable using electronic medical records and technology.
• Ability to travel to assigned facilities throughout the Atlanta region.
• Professional malpractice insurance.

Preferred Experience

• Behavioral health or psychiatric nursing experience.
• Experience working with geriatric patients.
• Assisted living, skilled nursing, or long-term care experience.
• Familiarity with collaborative care or behavioral health integration programs.

Position Details

• Pay: $30.00–$37.00 per hour
• Hours: Approximately 25–32 hours per week
• Schedule: Flexible, based on patient and facility needs
• Work Arrangement: Hybrid – Remote work with required visits to assigned facilities
• Service Area: Atlanta, GA and surrounding communities
