Case Manager

Mission Ministries Alliance improves the lives of people affected by homelessness,

providing emergency shelter, housing services, counseling, rehabilitation, and crisis assistance.

Position Purpose:

The Case Manager (Shelter, Transitional Living House) will contribute to the mission of MMA by providing case management and engagement to individuals experiencing homelessness with SUD/COD and living in a shelter and Transitional Living House.

Essential Job Duties and Responsibilities:

· Using discretion and independent judgment, develops strategies for engaging with clients, creating awareness about the program in the community, and developing a path to recovery.

· Participates in the assessment, evaluation, and planning of emergency shelter residents.

· Assess clients for level of care and make appropriate referrals to detox, inpatient treatment, or MAT when appropriate.

· Develops individualized person-centered case plans with SMART goals.

· Maintains positive working relationships with partner agencies.

· Coordinates and initiates services with other public and private agencies providing services to the homeless population.

· Provides referrals to agencies and service providers based on goals and objectives mutually agreed upon with the clients

· Establishes relationships with community partners and maintains a working relationship and knowledge of community resources.

· Follow up with all referrals to ensure that clients receive and access necessary services promptly.

· Utilizes the appropriate consents and documents accurately and timely with an understanding of the legal implications.

· Maintains training and license in HMIS data system. Maintains updates in the data system and other requirements of the data system.

· Participates in educational and required training of the agency.

· Participates in Coordinated Entry case conferencing and advocates for clients’ housing needs. Determining eligibility for all housing programs; Section 8, PSH, RRH, etc.

· Completes HART assessments for all clients who are in the shelter after 2 weeks of intake. Completes HART for all unsheltered clients when engaged within the community.

· Provides accurate reports to supervisor.

· Participates in agency staff meetings.

· Other duties as assigned.

Qualifications and Skills:

· Experience and/or substance use clinical licensure or certification.

· CADC preferred.

· Three years of experience working with SUD population.

· Lived experience preferred.

· Valid NC Driver’s License required.

Equipment:

Computers, various software programs, telephones, copiers, printers, and other office equipment. Basic tools such as hammer, screwdriver, drill, etc. Motor vehicle.

Relationships:

· Works closely with MMA clients.

· Works closely with all staff to support the mission of the organization.

· Works with community stakeholders to identify and provide resources for clients.

Schedule:

· Typical schedule is Monday through Friday from 8 AM to 5 PM with some evenings and weekends required.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
