Case Manager - IOP (Part-Time)

Overview

As a part-time Case Manager in an Intensive Outpatient Program (IOP), you’ll coordinate person-centered care for clients navigating mental health conditions or substance use disorders. You’ll support intake, assessment, care planning, and transitions while collaborating with clients, families, clinicians, and community providers. Your impact: Clear coordination and discharge planning help clients and families connect with appropriate care and community resources.

Responsibilities

• Complete intake and patient assessments; support diagnostic evaluation and clinical case management within your scope.
• Coordinate behavioral health services, referrals, community resources, and discharge plans.
• Communicate with clients, families, care teams, and managed care partners; document services in electronic medical records (EMR/EHR).
• Support continuity of care across outpatient, inpatient, clinic, acute, and post-acute settings, as applicable.
• Protect confidential medical records and follow Health Insurance Portability and Accountability Act (HIPAA) requirements.

Skills

• Relevant strengths include case management, social work, mental health counseling, addiction counseling, utilization management, patient care, patient service, client communication, task prioritization, and working with families and diverse cultural backgrounds.
• Knowledge of medical terminology, physiology, patient assessment, electronic health records (EHR) management, and community resource coordination in health services social work is valuable.
• Experience with children, people with disabilities, and individuals with mental health conditions or substance use disorders is relevant; pediatrics experience is a plus.
• Hospital, acute care, hospital medicine, emergency medicine, intensive care unit (ICU), pediatric ICU (PICU), operating room (OR), inpatient, outpatient, or clinic experience is helpful. Level I or Level II trauma center experience is also welcome.
• Basic life support (BLS) and cardiopulmonary resuscitation (CPR) knowledge or credentials may be relevant to the care setting.

Pay: $20.00 - $22.00 per hour

Expected hours: 15.0 – 30.0 per week
