Case Manager - Mental Health/SUD

We areis seeking an experienced Case Manager to join our team supporting facilities across Orange County. This role is responsible for coordinating client care, connecting individuals with appropriate services, and ensuring continuity of support across our locations.

Responsibilities

• Conduct client assessments and develop individualized care/service plans
• Coordinate with internal staff, healthcare providers, and outside agencies to arrange services
• Monitor client progress and adjust plans as needed
• Maintain accurate, timely documentation and case records
• Advocate for clients and connect them with community resources
• Ensure compliance with relevant regulations and facility policies

Requirements

• Prior case management experience required
• Experience working in a SUD (substance use disorder) or mental health treatment setting required
• Strong organizational and communication skills
• Ability to manage a caseload across multiple facilities in Orange County
• Valid driver's license and reliable transportation (for facility travel)
• Proficiency with Kipu documentation software

Location: Orange County

Pay: $23.00 per hour
