Role: LMSW Case Manager

Responsibilities:

• Conduct psychosocial assessments for inpatient members
• Develop care and discharge plans tailored to patient needs
• Coordinate services across medical, behavioral health, and community resources
• Collaborate with healthcare professionals to facilitate safe transitions of care
• Support patient and family understanding of treatment plans
• Reduce readmissions through effective care management
• Ensure accurate documentation and proficiency with EHR systems
• Commit to continuous improvement and patient-centered care
