Case Manager/ Social Worker

Responsibilities

• Conduct thorough patient assessments, including psychosocial evaluations and medical history reviews, to inform individualized care plans.
• Coordinate discharge planning by collaborating with healthcare teams, families, and community resources to ensure smooth transitions from inpatient to post-acute or outpatient settings.
• Manage clinical case documentation while maintaining compliance with HIPAA regulations.
• Provide behavioral health support and mental health counseling for members with diverse conditions, including mental health diagnoses.
• Facilitate communication between patients, families, healthcare providers, and community agencies to optimize care delivery and resource utilization.
• Facilitate individual sessions, workshops and groups.

Qualifications

• Knowledge clinical case management principles.
• Experience working with children, individuals with disabilities, mental health conditions, or substance use disorders is highly desirable.
• Excellent client communication skills along with the ability to work effectively with diverse cultural backgrounds and family units.
• Ability to prioritize tasks efficiently while managing multiple cases.
• License required for this position.

Pay: $45,000.00 per year
