Case Manager - Supportive Housing (Re-Entry)

A nonprofit services organization in New York City is seeking a new Case Manager to join their team in Brooklyn, connecting people to necessary community services for recently released individuals.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: 9am to 5pm
• Setting: Supportive housing program

Responsibilities:

• Providing case management services
• Developing and monitoring support plans
• Assisting with problem-solving and crisis intervention
• Accompanying consumers to appointments
• Acting as an advocate for consumers
• Performing other duties, as needed

Qualifications:

• 1+ year of related work experience
• High School Diploma / GED
• Solid assessment and documentation skills
• Patient oriented
• Great interpersonal and communication skills
• Strong attention to detail
• Highly organized

Desired Qualifications:

• 2 years of experience with Behavioral Health populations
• Bachelor's Degree
• Previous experience in a Supportive Housing and/or Scattering Site setting
• Solid understanding of Supportive Housing dynamics
• Experience with AWARDS
