Cornerstone – Case Manager

Cornerstone empowers people living with mental health and co-occurring substance use disorders to live, work, and integrate successfully into the community. Responsibilities of the Case Manager

• Develops an Individual Rehabilitation Plan (Treatment Plan) with the person served to identify case management needs in the areas of advocacy, coordination of services and resource procurement.
• Documents communication and interactions with the person served in progress notes in a timely manner. Assures that the assigned cases and all documentation are in compliance and accurate according to agency, COMAR, and CARF standards.
• Schedules and conducts home visits with persons served to monitor their condition and to assess their needs at least twice per month.
• Provides linkages and access to the necessary resources, entitlements and supports to ensure successful community integration.

Qualifications of the Case Manager

• A Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is required.
• At least one year of experience working in the human services field preferred
• Candidate must have a clean driving record with no more than two points and the ability to pass a criminal background
