Case Manager/Care Coordinator

Rapidly expanding aging service provider seeks part time Case Managers/Care Coordinators for its Aging Services program in Champaign and Piatt counties.

Duties and Responsibilities:

• Performing in-home comprehensive assessments for MCO participants
• Providing linkage and referral to other social service organizations
• Follow up activities, etc.
• Adherence to program timeframes and policies a must.

Qualifications and Skills:

• Candidate should possess a bachelor's degree in social science or health field or be a registered nurse**.** Formal work experience with the 60+ population can be substituted for bachelor's degree. One year of experience for one year of college, up to four years.
• Master's degree preferred
• Positions work out of the Urbana office
• Extensive travel and dependable transportation with vehicle insurance required
• Valid Driver’s license
• Computer literacy
• Strong organizational skills and the ability to work independently
• Bilingual candidates preferred
