Case Manager/Residential Rehab Program

Case Manager needed for busy NE Baltimore City Psychiatric Rehab Program.

Primary population are adults with severe and persistent mental illness/substance use disorders. Position provides wellness education, socialization activities and treatment plan interventions. Needs to be able to document observations as well as complete rehabilitation plans and daily/monthly encounter/progress notes. Functions as a clinical team player. Also creates/facilitates linkages with health providers as well as primary benefits programs. Position is FULL TIME. Must be able to pass a criminal background check as well as possess a valid Maryland drivers license. Position requires occasional transportation duties. Minimum of AA degree required and one year of related experience.

Pay: $20.00 per hour

Expected hours: 40.0 per week
