Case Planner

A nonprofit human services organization in New York City is seeking a Case Planner for a field-based role in Brooklyn, supporting children and families involved in foster care through case planning, family visits, court coordination, and ongoing service support.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday to Friday (with occasional Saturdays, as needed)
• Hours: Flexible daytime hours, generally between 8:00 AM and 7:00 PM
• Setting: Field

Responsibilities:

• Develop and update service plans in partnership with children, biological families, and foster families
• Conduct frequent field visits to foster homes, schools, medical providers, courts, hospitals, and other required locations
• Monitor child safety, well-being, and permanency progress through regular case contacts and documentation
• Coordinate supervised visits, family team conferences, and court appearances, including providing testimony when needed
• Collaborate with supervisors, nurses, child advocates, and other service providers to support case goals and family reunification

Qualifications:

• 1+ year of Case Management experience
• Bachelor's Degree
• Experience working with the Foster Care, Mental Health, and/or Child Welfare population
• Ability to work flexible hours, including some evenings as needed

Desired Skills:

• Bilingual (English and Spanish)
• Experience with Connections or ASARA
• Proficiency with Microsoft Office, including Outlook and Excel
