Case Worker

MINIMUM QUALIFICATIONS: Master of Social Work degree and three years of case management experience or Bachelor of Arts with a social science major (preferably Social Work or Sociology) and five years of case management experience.

Role: Under the general supervision of the Program and Assistant Directors, to provide case assistance to link senior clients with appropriate entitlement, benefits, programs and other resources to assist them with problems of daily living.

JOB DESCRIPTION:

• Works with Program Director and Assistant Director on relevant casework management issues.
• Completes eligibility screenings of benefits and entitlements.
• Assist members in applying for appropriate benefits and entitlements when they are eligible, or to obtain re-certification for entitlements.
• Assist in a casework advocacy role for resources and services in the community to help clients that require personal representation or direct intervention, as well as perform telephone reassurance.
• Maintain current resource files enumerating service/resource providers available for seniors in the larger New York area. May include provisions of short and long-term counseling services.
• Data entry of all client services into STARS pertaining to social work.
• Provides information and referrals, case assistance, telephone reassurance, and social outreach calls.
• Enters data appropriately into the NYC Aging database system and all written logs.
• Provides Database Manager with weekly logs and requested files.
• Completes light office tasks when not assisting members such as but not limited to: filing, registering new members, community outreach, making photocopies, etc.
• Attends necessary meetings and trainings, as directed.
