Context statement
The internal Assertive Outreach Service supports people and families who are homeless and sleeping rough in Alice Springs. The four-person team works

in community settings and at times that improve engagement, including early mornings and some work outside normal business hours. Three Case Workers

  2. Build respectful and persistent relationships with people sleeping rough, recognising individual choice, cultural context, strengths and readiness to

  1. Complete culturally appropriate intake, triage and needs assessments within training, role scope and established procedures, escalating complex or

high-risk matters.

  1. Provide practical assistance by accompanying or transporting clients to available accommodation, housing appointments, health services and other

  1. Provide practical assistance and supported access to identification, income, health care, transport, accommodation, housing applications and other

agreed supports.

  1. Work with clients and program manager to develop, implement and review case plans focused on stabilisation, future tenancy readiness and pathways

to safe, stable housing.

  1. Make warm referrals and support handovers to Lutheran Central Intake, housing, health, mental health, AOD, disability, justice, family safety, Centrelink

and community services.

  1. Participate in case conferences, joint outreach and coordinated service responses while maintaining clear professional boundaries.

  2. Maintain accurate and timely case notes, contacts, referrals, consent records, case-plan actions, outcomes and Specialist Homelessness Services

Collection-compatible service data.

  1. Monitor and communicate changes in safety, wellbeing, vulnerability and housing circumstances, and immediately escalate critical incidents, child-

safety concerns and serious risks.

  1. Seek and respond to service-user feedback and contribute observations about unmet need, service gaps and opportunities for improvement.

  2. Maintain vehicles, equipment and outreach resources in a safe and service-ready condition and comply with departmental policies and lawful directions.

Selection criteria
Essential

  1. Experience engaging and supporting people experiencing homelessness, disadvantage or complex needs in community or human-services settings, or

transferable experience that demonstrates the ability to do this work.

  1. Ability to build rapport through respectful, persistent and non-judgemental practice while maintaining professional boundaries.

  2. Ability to work safely and respectfully with Aboriginal people, families and communities and adapt practice to cultural and individual needs.

  3. Sound assessment, problem-solving and organisational skills, including the ability to identify risk, prioritise immediate needs and seek Managerial

guidance when required.

  1. Ability to contribute to housing-focused case plans and coordinate practical assistance and supported referrals across a range of services.

  2. Clear written and verbal communication skills, including the capacity to maintain accurate case records and service data.

  3. Ability to work effectively in a mobile team, participate in case reviews, manage competing priorities and contribute to a safe and supportive workplace.

Desirable

  1. A Certificate IV, Diploma or higher qualification in community services, mental health, alcohol and other drugs or a related field.

  2. Experience in mobile outreach, homelessness, housing, health, justice, disability or family support services.
