Catholic Charities Case Manager

JOB SUMMARY:

This role will work with, and advocate for, older adults (age 60+ and 18+ with disabilities) who are struggling to maintain independent living in their community. The caseload is primarily designated by MOHCD. They will also assist with rental assistance, rent payments, signing of rental agreements, etc.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Provide screening, clinical assessment, case management services, home visits, for rental subsidy program.

• Provide collaborative treatment planning and linkage services.

• Communicate with property management for client support.

• Ongoing coordination of support services and resource development.

• Conduct outreach in the community.

• Maintain electronic medical records.

• Follow all agency protocols, participate in all required agency meetings/trainings, and supervision.

• Maintain assigned caseload.

• Perform other duties as assigned.

JOB REQUIREMENTS:

• Bachelor’s degree in Social Work or relevant equivalent.

• 2-3 years of experience working with older adults is preferred.

• Ability to complete client home visits.

• Ability to work with a diverse range of clients with multiple needs.

• Proof of negative or inactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.

• Must be a self-starter with excellent follow-through skills.

• Willingness to work in a community-based setting in the Tenderloin neighborhood with low-income, frail and culturally diverse older adults who may have multiple behavioral and physical health problems.

• Ability to work with a team of diverse and multi-disciplinary professionals.

• Ability to maintain a professional demeanor with good interpersonal and communication skills.

• Ability to be flexible in the work environment.

• Work with translation services when needed.

• Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.

• Strong interpersonal, communication and organizational skills. Solid aptitude for customer service.

• High degree of skill in grammar, spelling and punctuation.

• Ability to maintain the confidentiality of all information obtained.

• Strong organizational skills and the ability to prioritize workflow.

• High level of analytical problem-solving skills.

• Excellent time management skills, including the ability to manage self-development.

• Must be action-oriented and have the ability to work independently and as a team player.

• Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.

PHYSICAL REQUIREMENTS/DEMANDS:

The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
