Certified Home Health Aide

Certified Home Health Aide (CHHA/CNA) – Hospice

Lakewood Hospice and Palliative Care, Inc. is seeking a compassionate and dependable Certified Home Health Aide (CHHA/CNA) to provide hands-on care and support to hospice patients throughout the Denver metro area and surrounding communities.

The ideal candidate has hospice experience and understands the importance of providing personal care with dignity, patience, and compassion.

Qualifications

• Current Colorado CNA/CHHA certification as required - Hospice experience preferred

• Home health, senior care, long-term care, or related experience welcomed

• Compassionate and respectful approach to patient care

• Dependable, professional, and comfortable working independently in patients’ homes

• Valid driver’s license and reliable transportation Responsibilities

• Assist patients with activities of daily living and personal care

• Provide bathing, grooming, dressing, toileting, and other appropriate supportive care

• Assist with repositioning, mobility, and comfort measures

• Observe and report changes in patient condition to the nursing team

• Provide companionship and emotional support

• Maintain a safe, clean, and comfortable environment for patients

• Document care appropriately and communicate with the hospice team Employment:

Full-time, part-time, or flexible combination
