Certified Nurse Practitioner

Job Summary

We are seeking a compassionate, dependable, and experienced Nurse Practitioner to join our primary care practice. The Nurse Practitioner will provide comprehensive, patient-centered care to adult patients, including preventive care, chronic disease management, acute illness evaluation, medication management, and patient education.

The ideal candidate is confident working independently while collaborating closely with physicians, clinical staff, specialists, and other members of the healthcare team. Duties

• Conduct thorough physical examinations and patient assessments, utilizing your knowledge of anatomy, physiology, and medical terminology.
• Prescribe medications and treatment plans in accordance with HIPAA regulations and collaborative practice agreements.
• Utilize electronic health record (EHR) systems like Practice Fusion to document medical records accurately and efficiently.
• Collaborate with healthcare professionals across disciplines to develop comprehensive discharge plans, case management strategies, and utilization reviews.
• Ability to develop individualized treatment plans and monitor patients for effectiveness, side effects, and adherence.
• Order, review, and interpret laboratory tests, imaging studies, EKGs, pulmonary function testing, and other diagnostic procedures.

Requirements

• Valid Nurse Practitioner license with current certification to practice independently or under collaborative practice agreements.
• Proven experience working in diverse clinical environments such as hospital medicine, emergency medicine, ICU, or telehealth settings.
• Strong knowledge of EMR/EHR systems ; proficiency in CPT coding and ICD coding (ICD-10)
• Demonstrated ability to perform physical examinations, vital signs assessment, phlebotomy, venipuncture, and sterile technique.
• Excellent communication skills with the ability to educate patients on health coaching and medication administration.
• Background in adult primary care, geriatrics, or dementia care is preferred.
• Knowledge of Workers' Compensation law and HIPAA compliance standards; experience working with individuals with disabilities or developmental delays is advantageous.

Pay: $80,000.00 - $120,000.00 per year
