Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and well-being
• Monitor and record vital signs including blood pressure, temperature, pulse, and respiration rates accurately in electronic health record (EHR) systems
• Support patient transfer methods using equipment like Hoyer lifts and manual handling techniques to ensure safe mobility
• Administer medications under supervision following strict protocols and maintain detailed medical documentation in compliance with HIPAA regulations
• Observe and report changes in patient condition, including symptoms related to dementia, Alzheimer's disease, or other cognitive impairments
• Provide specialized care for patients with complex needs such as those requiring memory or behavioral health support
• Collaborate with multidisciplinary teams in settings like long-term care facilities, hospitals, or outpatient clinics to deliver seamless patient care

Skills

• Proven experience working with diverse populations including geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care
• Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks for accurate medical records management
• Knowledge of medical terminology, aseptic techniques, and basic life support (BLS) procedures including CPR and first aid
• Ability to perform phlebotomy and catheter care when needed; experience with medication administration is a plus
• Strong communication skills for effective caregiver-patient interaction and teamwork within fast-paced healthcare environments like ICU or trauma centers
• Physical stamina for heavy lifting, safe lifting practices, and manual handling during patient transfers or mobility assistance
• Experience working in specialized settings such as emergency medicine, hospital medicine, pediatrics, or sub-acute care environments

Pay: $25.00 - $26.00 per hour
