Job Summary

Duties

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote comfort and dignity
• Monitor vital signs including blood pressure, pulse, respiration, and temperature, accurately documenting all findings in electronic health records (EHR) systems
• Support patient transfer methods using equipment like Hoyer lifts and ensure safe manual handling practices at all times
• Provide basic medical care such as wound care, catheter care, and medication administration under supervision, adhering to aseptic techniques and infection control protocols
• Observe and report changes in patient condition promptly to nursing staff or medical professionals
• Maintain patient confidentiality by following HIPAA guidelines and ensuring secure handling of medical records and documentation
• Assist with patient transfers, mobility exercises, and positioning to prevent complications and enhance comfort

Requirements

• Valid certification as a Certified Nursing Assistant (CNA) in accordance with state regulations
• Experience working with diverse populations including geriatrics, individuals with developmental disabilities, dementia or Alzheimer’s disease, and patients requiring memory or hospice care
• Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation
• Knowledge of medical terminology, vital signs measurement, and basic anatomy relevant to patient care
• Ability to perform heavy lifting safely using proper lifting techniques and assistive devices like Hoyer lifts
• Strong communication skills for caregiving interactions and teamwork within multidisciplinary healthcare settings
• Prior experience in long-term care facilities, hospitals (including ICU or trauma centers), outpatient clinics, or home & community care environments is highly desirable
• Certification in Basic Life Support (BLS) or CPR is required; additional certifications such as phlebotomy or DME experience are a plus
