Job Summary

Duties

• Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and dignity.
• Monitor and record vital signs including blood pressure, pulse, respiration, and temperature accurately using electronic health record (EHR) systems.
• Support medical teams by preparing patients for examinations or procedures, including assisting with phlebotomy and catheter care.
• Maintain strict adherence to infection control protocols and aseptic techniques to ensure patient safety.
• Document patient observations and care provided in accordance with HIPAA regulations and medical documentation standards.
• Operate patient transfer methods such as Hoyer lifts safely to prevent injury during mobility assistance.
• Communicate effectively with patients, families, and healthcare professionals to coordinate care plans and address patient needs promptly.

Experience

• Prior experience working with geriatric patients in long-term care facilities or nursing homes is highly desirable.
• Demonstrated knowledge of medical terminology, vital signs monitoring, and basic life support (BLS).
• Experience working with individuals with developmental disabilities or dementia care is preferred.
• Familiarity with electronic health records (EHR/Epic/Cerner/eClinicalWorks) systems for accurate medical documentation.
• Previous roles involving hospital medicine, ICU, emergency medicine, or post-acute care provide valuable insight into acute and critical care environments.
• Hands-on experience with patient transfer methods, manual handling, heavy lifting, and safe lifting practices is essential.
• Knowledge of infection control practices specific to infectious disease care or hospice settings enhances your ability to deliver safe patient-centered care.

Pay: $25.76 - $27.00 per hour
