Position Summary

Eleanor Home Health Agency is seeking a compassionate, dependable Certified Nursing Assistant (CNA) to accompany senior clients to and from medical and other scheduled appointments. This position provides personalized support to help clients travel safely, navigate appointments, and return comfortably to their residence or care facility.

This is a ride-along escort position—not a driving position. Transportation will be provided, and the CNA will not be responsible for driving or using a personal vehicle to transport clients.

The ideal candidate is patient, attentive, and committed to treating older adults with dignity and respect.

Key Responsibilities

  • Accompany clients throughout the assignment: Meet clients at the designated pickup location, ride with them to their appointments, remain available during the visit, and accompany them on the return trip.
  • Provide safe mobility assistance: Help clients with walking, wheelchairs, walkers, and transfers—including entering and exiting the vehicle—as appropriate to the client’s needs, the employee’s training, and the established care plan.
  • Assist with appointment logistics: Help clients locate the correct office, check in, manage necessary paperwork, and keep track of personal belongings and mobility aids.
  • Offer companionship and reassurance: Provide calm, respectful support, particularly for clients who experience anxiety, confusion, memory loss, or difficulty navigating unfamiliar settings.
  • Provide personal assistance as needed: Assist with permitted activities of daily living, such as toileting, within the assigned care plan while maintaining privacy and dignity.
  • Communicate and document: With appropriate consent and authorization, take notes or relay appointment instructions to the designated family member or agency supervisor without independently interpreting medical advice. Document services provided and promptly report delays, safety concerns, or changes in the client’s condition.
  • Support a safe return and handoff: Follow the client’s supervision plan and confirm the appropriate return or handoff arrangements before ending the assignment.

Qualifications

  • Current CNA certification or active nurse aide registry status, as applicable.
  • Experience assisting older adults in home care, assisted living, nursing facilities, or a similar setting.
  • Strong communication skills, sound judgment, punctuality, and a professional demeanor.
  • Ability to assist safely with mobility and personal care, with or without reasonable accommodation, consistent with training and assigned client needs.
  • Ability to complete required background checks and agency onboarding.

Preferred: Experience supporting clients with dementia, wheelchair users, or individuals who need additional assistance during appointments. CPR and First Aid certification are preferred.

Working Conditions and Scope

Assignments may involve travel as a passenger, walking within healthcare facilities, and waiting during appointments. Employees must remain attentive and available for the duration of the assigned escort service.

This role focuses on non-medical escort support and personal assistance. It does not include driving, independent medical decision-making, medication administration, or skilled nursing procedures. Client concerns and emergencies must be handled according to agency procedures.
