CFS CASE WORKER

Curated Support Services LLC is seeking a compassionate, organized, and dependable Coordinated Family Support (CFS) Case Worker to support adults with developmental disabilities and their families throughout Orange County, California.

The CFS Specialist will provide individualized service coordination, resource navigation, advocacy, and community support to help individuals maintain stability, increase independence, and remain connected to their families and communities.

Job Responsibilities

• Coordinate individualized supports and services based on each person's needs, preferences, and goals.
• Assist individuals and families with accessing community and generic resources, including SSI, Social Security, Medi-Cal, IHSS, CalFresh, transportation, housing, and other community programs.
• Assist with applications, renewals, referrals, documentation, and follow-up with government agencies and community organizations.
• Coordinate medical, dental, behavioral health, educational, vocational, and personal appointments.
• Connect individuals to community activities, social programs, educational opportunities, volunteer opportunities, and other meaningful activities.
• Support individuals in developing practical skills related to budgeting, organization, communication, social interaction, community participation, and independence.
• Assist individuals and families with future planning related to living arrangements, services, employment, education, community involvement, and long-term stability.
• Establish measurable goals and monitor progress toward identified outcomes.
• Identify gaps in services and help coordinate appropriate referrals and supports.
• Provide advocacy and assistance when individuals or families encounter barriers accessing needed services.
• Maintain professional communication with individuals, families, providers, and community partners.
• Complete timely and accurate progress notes, service documentation, reports, referrals, and follow-up records in accordance with CSS and Regional Center requirements.
• Maintain confidentiality and comply with applicable policies, regulations, and program requirements.
• Travel throughout Orange County to meet with individuals, families, providers, and community partners as needed.

Qualifications

• Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Rehabilitation, Developmental Disabilities, or a related field preferred.
• Minimum of one year of experience working with adults with developmental, intellectual, cognitive, or physical disabilities.
• Previous experience in case management, service coordination, advocacy, or social services strongly preferred.
• Knowledge of SSI, Social Security, Medi-Cal, IHSS, CalFresh, and other community resources.
• Familiarity with the California Regional Center system is preferred.
• Strong written and verbal communication skills.
• Excellent organization, time management, and follow-through.
• Ability to work independently and manage multiple priorities.
• Ability to build positive and professional relationships with individuals, families, and service providers.
• Strong problem-solving and advocacy skills.
• Ability to remain calm, professional, and solution-focused when addressing challenging situations.
• Valid California driver's license, reliable vehicle, and current automobile insurance required.
• Must successfully complete a criminal background check prior to employment.
• Must be willing to travel throughout Orange County.
• Spanish or other bilingual/multilingual skills are preferred.

Pay: From $23.00 per hour
