The Role:

We are seeking a proactive and organised Care Coordinator to join our dynamic team. This is a hands-on coordination role responsible for ensuring participant supports are delivered safely, consistently and in line with NDIS standards.

Key Responsibilities:

• Coordinate SIL clusters and assigned community participants.
• Actively engage with Participants and respond to changes in service needs and communicate changes with support workers.
• Ensure support plans and documentation remain current.
• Balance participant support needs with workforce sustainability.
• Work closely with rostering team to ensure shifts align with participant needs.
• Coordinate diaries and manage scheduling commitments.
• Liaise with families, allied health professionals and stakeholders.

About You:

• Experienced in disability or community services (desirable, but not essential).
• Proficient in Microsoft Office Suite and data entry.
• Current driver's license and access to a personal vehicle.
• Calm under pressure in a 24/7 support environment.
• Comfortable managing complex, high-intensity support environments.
• NDIS Worker Screening Check or willingness to obtain one.
• Full working rights in Australia.
