Client Care Coordinator

We are a growing home care agency seeking a professional, compassionate, and organized Care Coordinator to help manage and elevate our client experience.

This role is ideal for someone who enjoys working with people, building relationships, and ensuring clients feel supported, heard, and well cared for.

This is a hands-on role that involves both office coordination and in-field client interaction.

About the Role:

As a Care Coordinator, you will be responsible for:

• onboarding new clients
• maintaining ongoing client relationships
• ensuring quality of care
• and acting as a key point of contact for client needs

You will play a major role in making sure nothing falls through the cracks.

Key Responsibilities:

• Conduct in-home consultations (when nurse is unavailable)
• Gather client information and assess care needs
• Set up and maintain client files (digital + physical)
• Ensure all documentation is complete and audit-ready

Client Follow-Up & Relationship Management

• Conduct ongoing client check-ins
• Build strong relationships with clients and families
• Address concerns professionally and proactively
• Compliance & Documentation
• Maintain organized, up-to-date client files
• Ensure all records meet compliance standards

Field Visits & Quality Assurance

• Perform client visits as needed
• Identify and address any issues in the home
• Provide feedback to office team on client needs

Team Collaboration

• Communicate client needs to scheduling and recruiting team
• Help ensure proper caregiver matching
• Support overall client experience and retention

• Strong communication and people skills
• Professional, calm, and solution-oriented
• Highly organized and detail-oriented
• Ability to handle difficult conversations with confidence
• Team player with a positive, proactive attitude

Preferred Background:

• Healthcare, home care, or social work
• OR strong customer service / client-facing experience
• Sales or relationship-building experience is a plus

(We are open to candidates who are quick learners and highly motivated)

Requirements:

• Reliable transportation
• Valid driver’s license
• Ability to travel locally for client visits

Compensation:

• Competitive hourly rate (based on experience)
