Client Care Coordinator

Own the day-to-day client experience for South West Georgia Homecare. You will run intakes, match caregivers to families, build the schedule, and be the named person families can reach directly.

Full-time Albany, GA Care Operations Based on experience

What you will do

• Complete client intakes and in-home service assessments
• Match caregivers to clients by skills, personality, and proximity
• Build and maintain the weekly caregiver schedule
• Serve as the named point of contact for families
• Coordinate hospital discharge and referral requests
• Document service changes and escalate condition changes same day

What you need

• Two or more years of scheduling, care coordination, or healthcare operations experience
• Excellent phone and written communication with families
• Comfort working in scheduling and electronic record systems
• Strong organization under shifting daily priorities
• Knowledge of Dougherty, Lee, and surrounding counties preferred
• Ability to pass background screening
