ABOUT US
Wyoming Guardianship is a dynamic social services organization serving Wyoming residents with disabilities. Working in this field is both rewarding and challenging and requires an individual with tenacity, passion, and trustworthiness. This is an opportunity to truly impact the lives of the clients you work with.
If you are interested in working with an organization that has learning & growing opportunities, a very flexible work environment, and working remotely - we may be the perfect fit for you! Our organizational culture revolves around flexibility, transparency, honesty and the ability to dress for your day. Our organization offers a generous amount of PTO and 12.5 paid holidays per year! WGC also offer a supplemental cafeteria plan for health benefits through Aflac, Vision and Dental (free to employees), an EAP as well as a Simple IRA retirement program for those that meet the criteria.
About the Work
In joining our team as a Client Support Coordinator, you will have a unique opportunity to directly make a difference in the lives of our clients by connecting with them and making life decisions for whom WGC serves as guardian/care manager while keeping with the National Guardianship Association (NGA) Standards of Practice and Ethical Guidelines and Wyoming Statute.
This position is a remote working position. You have the autonomy and ability to work from your home office. WGC will provide computer, cell phone and office supplies. Travel is required (estimated at 50% of your time) to visit clients on a monthly basis and travel is determined based on where we hire at without the state of Wyoming and where the client caseload will be. An on-call rotation is required every 6 weeks.
This position is part of a fully grant funded contract with the Department of Family Services as part of the Youth Transition Program. The CSC will be working with vulnerable adults ages 18 through 21 that are in need of guardianship. This position operates under WGC's contract with DFS and its accompanying statement of work, making decisions for the person under guardianship in alignment with WGC policies, Wyoming statues and the National Guardianship Association (NGA) Standards of Practice and Code of Ethics, and the terms of the DFS Contract.
This position ensures that all decisions and actions promote the individual’s well-being, rights, dignity, and least restrictive alternatives, while maintaining accountability to WGC as the legal guardian and to DFS as the contracting agency. Because this population is transitioning from youth- and family-based systems into adulthood, the CSC pays particular attention to continuity of care, connection to adult service systems, and coordination with DFS/APS caseworkers throughout the guardianship.
A Day in the Life:
- Exercise delegated decision-making authority on behalf of WGC for individuals under guardianship, ensuring decisions are person-centered, promote the ward’s dignity and independence, and are in the individual’s best interest
- Conduct required monthly contact with each assigned individual through in-person visits. Participate in care planning, treatment team meetings, medication management meetings, and other interdisciplinary meetings to obtain information, communication concerns, and advocate for clients.
- Coordinate and collaborate with DFS/APS caseworkers and external providers and professionals with particular attention to services supporting the transition from youth- and family-based systems into adult service systems
- Document interactions and billable time with your client or any of their providers/team in WGC's database and provide documentation to WGC to ensure timely semi-annual court reporting.
- Collaborate as a team and under the guidance of the Director of Life Management Service
