Clinical Case Coordinator

Overview
We are seeking a dedicated and experienced Case Coordinator to join our healthcare team. The ideal candidate will play a crucial role in managing patient care from admission through discharge, ensuring that patients receive the appropriate services and support throughout their healthcare journey. This position requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with various healthcare professionals.

Duties

• Receives case referrals. Reviews available patient information related to case, including disciplines required to determine home care needs. Assigns appropriate clinicians to case.
• Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verified diagnosis, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as needed.
• Reviews patient's clinical diagnoses, medications, procedures, and clinical course.
• Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
• Coordinates and communicates with patients, caregivers, and legal representatives regarding plan of care and patient's status and reports to physicians when necessary.
• Attends case conference meetings with organization personnel to facilitate coordination of care.
• Participate in quarterly record reviews and communicates findings and recommendations to Clinical Director.
• Assists in orientation of new clinicians.
• Complies with accepted professional standards and principles.
• Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
• Promotes professional customer service orientation to field clinicians.
• Consults with physician, nurses, social workers, therapists, discharge planners, and other disciplines to establist a coordinated home plan of care.
• Performs other duties and activities as delegated by the DPCS/Administrator.

Position Qualifications:

• Must be a Licensed nurse with current LVN licensure to practice professional nursing in the state of California.
• Graduate of an accredited school of nursing.
• Two years experience within the past 5 years in a home health setting, primary care clinic, or healthy facility.
• Knowledge and proficiency in clinical decision making.
• Complies with acceptable professional standards and practice.
• Has an active CPR Certification.
• Proficiency in discharge planning processes and medical records management.
• Strong understanding of patient service principles and managed care systems.
• Ability to perform intake assessments and engage in utilization management practices.
• Excellent communication skills for effective collaboration with healthcare professionals and patients alike.
• Familiarity with case management principles and practices within both inpatient and outpatient settings.
• Demonstrates excellent observation, verbal, and written communication skills.

Job Type: Full-time

Pay: $27.00 - $30.00 per hour
