Clinical Coordinator - Waiheke Island

We’re looking for a skilled Registered Nurse with strong ACC experience to support our diverse range of clients living in Waiheke Island to join our team. We are a not-for-profit organization providing Home and Community-based Nursing Services across Northland, Greater Auckland, and Southland/Otago/Canterbury regions.

Essential:

• NZ Registered Nurse with a current Annual Practising Certificate (APC).
• Local New Zealand clinical experience (community, primary care, rehabilitation, ACC pathways, or related).
• Previous experience working with ACC processes, reporting, or service contracts.
• Strong clinical assessment skills and high attention to detail.
• Excellent written and verbal communication skills.
• Full New Zealand Driver Licence.
• Candidates must be based in Waiheke Island.

What will you do?

No two days are the same for a Clinical Coordinator- this position will keep you engaged by:

• Managing a caseload of clients
• Assessing clients using InterRAI assessment tools
• Identifying support and rehabilitation needs
• Developing individualised support plans to ensure the needs of the people we support, and their whanau are met with care
• Providing clinical oversight to our support workers
• Building relationships, identifying opportunities, collaborating with other healthcare providers and thinking outside of the box!
