Clinical Documentation Auditor

Key Responsibilities

• Review clinical notes, assessments, orders, and other patient records against established checklists and client requirements.
• Identify missing signatures, dates, supporting details, or inconsistencies in documentation.
• Communicate findings clearly to clinicians and internal teams for correction or clarification.
• Track outstanding documentation and follow up until issues are resolved.
• Maintain accurate audit logs and report recurring documentation concerns.
• Handle patient information confidentially and follow company procedures.

Qualifications

• Graduate of a healthcare-related course preferred.
• Experience reviewing home health or hospice documentation is an advantage.
• Strong attention to detail and written English communication skills.
• Able to organize multiple records and meet deadlines.
• Comfortable using electronic health records, spreadsheets, and email.
• Willing to work onsite on a night shift schedule.
