Job Overview

Divine Care Spaces is seeking a Registered Nurse to serve as the Clinical Nurse Supervisor for our small assisted living home in Brooklyn Park, Minnesota.

We are a residential-style assisted living facility with a maximum capacity of five residents. This position is ideal for an RN looking for a flexible part-time clinical leadership role in a smaller, more personal care setting.

The Clinical Nurse Supervisor will provide clinical oversight of resident care, medication management, nursing assessments, staff delegation and supervision, service plan updates, staff education, and other clinical responsibilities associated with Minnesota assisted living requirements.

Because of the small size of our facility and generally light clinical workload, this is not a traditional full-time nursing position and does not require continuous on-site nursing coverage. The RN will generally visit the facility once per week for routine clinical oversight and will have flexibility to complete appropriate clinical and administrative responsibilities remotely or off-site.

Additional on-site visits may occasionally be necessary depending on resident needs, admissions, assessments, changes in condition, staff competency needs, or other clinical circumstances.

This position is a strong fit for an RN with experience in assisted living, home care, long-term care, waiver services, behavioral health, or a similar setting who is comfortable working independently with a small care team.

Duties

Provide clinical oversight for a small assisted living facility in accordance with applicable Minnesota assisted living requirements.

Complete resident nursing assessments, reassessments, and focused assessments as required or clinically indicated.
Participate in pre-admission reviews and assessments as appropriate.
Review and update resident service plans based on resident needs, reassessment findings, and changes in condition.
Oversee medication management services, including medication documentation, medication-related concerns, orders, and follow-up.
Provide RN delegation, supervision, and evaluation of unlicensed staff performing delegated nursing tasks.
Train and educate staff regarding resident care needs, medication administration, delegated tasks, infection control, documentation, and other required clinical topics.
Provide phone or remote clinical consultation to staff regarding delegated nursing tasks, changes in condition, medication concerns, or other resident care issues.
Review resident documentation, incident reports, changes in condition, and clinical concerns as appropriate.
Assist with new admissions, discharges, service plan development, and ongoing resident care planning.
Communicate with residents, families, case managers, healthcare providers, pharmacies, and facility leadership when necessary.
Help ensure resident clinical records are accurate, organized, complete, and survey-ready.
Support compliance with assisted living regulations, facility policies, resident rights, medication management requirements, and safety standards.
Participate in quality improvement activities and identify opportunities to improve resident care, staff practices, or documentation.
Provide clinical guidance to direct care staff in a professional, supportive, and respectful manner.
Complete appropriate clinical, documentation, care coordination, and administrative nursing responsibilities remotely or off-site when permitted and appropriate.

Qualifications

Current Minnesota Registered Nurse license in good standing.

Experience in assisted living, home care, long-term care, waiver services, behavioral health, or a related setting preferred.
Knowledge of Minnesota assisted living requirements preferred.

Comfortable working independently in a small residential assisted living environment.
