Clinical Pharmacist

Job Purpose

To provide comprehensive clinical pharmacy services by optimising medication therapy, promoting medication safety and supporting evidence-based prescribing to achieve the best possible patient outcomes.

Job Responsibilities

• Conduct comprehensive medication reviews to assess the appropriateness, effectiveness and safety of patients’ medication therapy.
• Participate in multidisciplinary ward rounds and provide evidence-based recommendations on medication selection, dosing, interactions and therapeutic monitoring.
• Identify, resolve and document medication-related problems and clinical pharmacy interventions to enhance medication safety and patient outcomes.
• Provide medication counselling and education to patients, caregivers and healthcare professionals to promote safe and effective use of medications.
• Collaborate with Doctors, Nurses and other healthcare professionals on medication management, antimicrobial stewardship, medication safety and other clinical pharmacy initiatives.

Job Requirements

• Minimum Bachelor’s Degree in Pharmacy from a recognised institution.
• Registered Pharmacist with the Pharmacy Board Malaysia and possess a valid Annual Retention Certificate (ARC).
• Minimum 5 years working experience in private healthcare setting.
• Relevant experience in clinical pharmacy or a hospital setting is preferred.
• Strong knowledge of pharmacotherapy, medication safety, drug interactions and evidence-based clinical practice.
• Good analytical, communication and interpersonal skills with the ability to work effectively within a multidisciplinary healthcare team.
