Clinical Pharmacist

Job Summary
We are seeking a dedicated and highly skilled Clinical Pharmacist to join our healthcare team on a fixed term contract. The successful candidate will play a vital role in providing expert medication management and patient care within a clinical setting. This position offers an opportunity to work in a dynamic environment, supporting patient recovery and ensuring safe medication practices. Responsibilities

• Monitor medication reviews and reauthorisations accurately and efficiently, ensuring compliance with legal and safety standards
• Provide expert advice to healthcare professionals and patients regarding medication usage, interactions, and side effects
• Monitor patient responses to medication therapy and adjust treatment plans accordingly in collaboration with medical teams
• Maintain comprehensive documentation of medication administration and patient records
• Assist in the development and review of clinical protocols related to medication management
• Educate patients on proper medication use, storage, and potential adverse effects
• Participate in multidisciplinary team meetings to optimise patient care strategies

Experience

• Fully qualified Pharmacist registered with the General Pharmaceutical Council (GPhC) or equivalent regulatory body
• Proven experience working within a GP Practice environment is highly desirable but not essential
• Excellent communication skills, capable of liaising effectively with healthcare professionals and patients alike
• Ability to work under pressure whilst maintaining meticulous attention to detail
• Commitment to ongoing professional development and continuous learning within the pharmacy field

Pay: £29,138.66-£47,510.56 per year
