Clinical Pharmacist

• Review and validate medication orders and prescriptions for accuracy and appropriateness.
• Monitor drug therapy for efficacy, safety, adverse drug reactions, and potential drug interactions.
• Identify and report medication errors, adverse drug reactions (ADRs), and drug-related problems.
• Provide clinical pharmacy services, including medication reconciliation and medication history review.
• Participate in ward rounds and collaborate with physicians and nursing staff.
• Provide drug information and evidence-based medication recommendations to healthcare professionals.
• Counsel patients on medication use, dosage, administration, precautions, and possible side effects.
• Monitor high-alert medications, antibiotics, and other medications requiring special attention.
• Support antimicrobial stewardship and rational drug-use initiatives.
• Maintain accurate clinical pharmacy records and prepare relevant reports.
• Assist in medication safety audits, quality improvement activities, and hospital accreditation requirements.
• Follow hospital policies, clinical protocols, and applicable pharmacy regulations.

EXPERIENCE: Minimum 1 year

Pay: ₹300,000.00 - ₹600,000.00 per year
